Our specialist complex home care and support provide our Clients with professional, focussed care for a range of complex physical health needs and conditions. These can be long-term health conditions, end of life care, acquired injuries and neurological conditions, which includes acquired brain injury, spinal cord injury, motor neurone disease, multiple sclerosis and muscular dystrophy.
Our holistic person-centred approach means the complex home care we provide is tailored exactly to our Clients’ individual needs. We focus on ensuring our Clients can live as independently as possible, supporting each individual in many aspects of daily living and enhancing their quality of life.
We train our Carers in specialist areas, such as Epilepsy, PEG feeding, skin integrity, stoma care and tracheostomy care, amongst many others. A crucial member of our team is our Clinical Lead, a Registered General Nurse who provides this training, as well as providing clinical advice and best practice guidance to ensure our clients are receiving expert clinical care from properly trained and skilled professionals.
The clinical services we provide are overseen by our RGN Clinical Lead, they include:
- Respiratory management such as non-invasive ventilation, cough assist and oral suction
- Tracheostomy care including tracheal suction and changes
- Gastrostomy feeding
- Medication management
- Epilepsy management and Buccal Midazolam Administration
- Stoma and Catheter care
- Moving and positioning, including hoisting
Of course, we also assist our Clients with their daily living activities, such as
- Domestic tasks, such as cleaning and laundry
- Personal care
- Monitor food intake and nutrition
- Manage financial transactions
- Access the community and participate in activities such as shopping, further education, cinema and other general appointments
We meet with every individual to assess their needs, to understand their personality, their hopes, likes and dislikes, and to talk to their loved ones. We do this so we ensure we deliver the right complex home care package for every person.
Our Carers can support individuals from one hour per day, up to 24 hours per day, 365 days a year. This provision includes Out of Hours support for our Clients and Carers, which can be used to clarify minor queries, or it can be used to assist staff during a Client emergency.
